IT shop - remove 2 items:"valid from" and "valid to" in request form

IT shop - remove 2 items:"valid from" and "valid to" in request form

how  hide them from the request form ?

or how to make them to grey(no able to edit)?

I removed them from the overview table ,but not work please help .....

below highted are the items what I want to hide :

and I changed here(remove the 2 highlighted fields name, not work)
