Object used as template was not loaded from database

When trying to submit a form from webUI we are seeing the below error. 1IM v8.1.1

An exception has occurred while executing the form method Popup4_Popup4_ControlRef2_Container6_Button1_Method.

Object used as template was not loaded from database.

We compared to make sure all the processes and associated events exist for 'OnYes' in webdesigner. Any pointers on how to get to the bottom of this error will be greatly helpful

I suggest turning on the web portal debug log to get some more information about the error. Does the event generation work, if you test it in the Object Browser using the same set of credentials?
