You don't say what version you are using ..... but this applies to most versions ....... to send an email on deny you need to specify the mail template to use in the 'approval policy' linked to the 'attestation policy'.
The docs state:
Mail templates |
Mail template used to create email notifications for granting, denying, extending, unsubscribing, or canceling an attestation or for giving notice of its expiry. |
