Approvals are getting skipped all of a sudden

We have several workflows setup that have approval steps in them.   A tech adds a user to a certain group, they get a pop up for justification, that gets emailed to manager and then they approve or reject the request.  This has worked great for over a year.

We noticed today that for some reason, the approval step is now getting skipped altogther in every workflow that has an approval step.   We have several of these workflows, always worked great until recently.  Now a tech puts a user in any of these groups and no approval, the user just gets added to the group.  

We are on 8.1.3 sp1 and have been for a couple months now.  We've restarted services etc, this is happening no matter which AR server is connected to, we have several.

Has anyone else experienced this?  We've opened a ticket with support and sent logs waiting to hear back, just curious if anyone else has seen or heard of this issue.  Thanks.

    Does the Active Roles Event Log show any Workflow activity? Even if the "Enforce approval" Run As option is not enabled, and an Active Roles Admin adds someone to a group that is affected by an Approval Workflow, it still logs information that the "Workflow instance has been started". Something else worth checking, if the "Enforce approval" option is not enabled, could these techs have inadvertently been added to the group that grants them Active Roles Admin rights? Try enabling this option (if not already enabled) and see what happens.

    Also, if it is not that much trouble, try creating a new one (not making a copy of an existing one) and disabling the corresponding, existing Workflow and see if the new one works as expected.

    Yeah so I know the workflows are running because we have several other checks done on the object being added, for instance if its a PC, the techs will get a stop error telling them PCs are not allowed, those stop checks are still  working so the workflow is definitely being run, its just the approval step is getting skipped for some reason

    My thought around this is that possibly the logic that would normally allow AR Admins to bypass workflow approval steps has somehow kicked in here.

    Have you checked he properties of the workflow to ensure that the "Enforce Approval" checkbox is enabled?  This is supposed to ensure that Approvals are enforced for EVERYONE whether they are an AR Admin or not.
