There are lots of systems and lots of patches. If your organization follows best practices, then there should be a closed-loop patch management policy and vulnerability scanning process in place that would find patch failures. Even if your organization understands today’s risks and is committed to securely managing privileged accounts, it’s still not easy.
All it takes is one overlooked patch and hackers have an opening, but even so that shouldn’t be the only thing that hackers would need to find to get to your organization’s critical data.
